TOWN OF SHERBORN
RECREATIONAL AND ACTIVITIES RELEASE FORM
I, the undersigned _____________________, do hereby consent to my participation in recreational activities and programs of the Town of Sherborn.
I also agree to forever release the Town of Sherborn and all their employees, agents, board members, volunteers and any and all individuals and organizations assisting or participating in any voluntary or recreational activities and programs of the Town from any and all claims, rights of action and causes of action that may have arisen in the past, or may arise in the future, directly or indirectly, from personal injuries to myself or property damage resulting from my participation in the Town of Sherborn voluntary activities or recreational programs.
I acknowledge that participation in these voluntary activities may expose the participant to risks of personal injury or death resulting from such participation and the use of materials and equipment by participant and other participants, and the exposure to harm such as may be presented by the COVID-19 virus. I am aware and acknowledge that the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization, is extremely contagious, and is believed to be spread mainly from person-to-person contact. While the Town of Sherborn has put in place preventative measures in an effort to reduce the spread of COVID-19, I acknowledge that the Town cannot guarantee that I will not become infected with COVID-19, and I acknowledge that attending any recreational activity or program may increase participant’s risk of contracting COVID-19.
I also promise, to indemnify, defend, and hold harmless the Releasees against any and all legal claims and proceedings of any description that may have been asserted in the past, or may be asserted in the future, directly or indirectly, arising from personal injuries to myself or property damage resulting from participation in the Town of Sherborn voluntary activities or recreational programs.
I agree that this participation is at the discretion of the Town of Sherborn and if the participant becomes a discipline problem, he or she will be expelled from the program or activity without refund of the program fee.
I further affirm that I have read this Consent and Release Form and that I understand the contents of this form. I understand that my participation is voluntary and that I am free to choose not to participate in said activities. I also understand that my voluntary participation in an activity does not make me an official Town volunteer. By signing this form, I affirm that I have decided to participate in the Town in its recreational activities with full knowledge that the Releasees will not be liable to anyone for personal injuries and property damage that I may suffer in voluntary Town activities or recreation programs.
Additionally, I hereby grant permission for any photographs or video’s taken during this program or activity to be used in print or electronically, specifically for promotion matter, press, and/or future advertising.
